SOCIAL WORK SERVICES

Our dedicated Social Workers take a holistic approach to supporting individuals and/or their families during life journeys. They help people to navigate complex systems, including the NDIS, Healthcare and government agencies, to ensure people are provided with the supports that meet their individual needs.

The team are committed to empowering individuals and their caregivers to achieve goals and create stronger connections.

Social inclusion is a core aspect of our social workers’ roles with a view to fostering connections and building a sense of belonging.

ASSESSING AND PLANNING

Strengths and needs assessment (including comprehensive initial assessment to identify support needs).
Psychosocial assessment – learning about the participant’s difficulties, strengths and needs, to create therapy goals and plan capacity building, recovery and supports. The information allows us to learn more about the person beyond the diagnosis.
NDIS plan review reports.

COORDINATION

Engaging and facilitating partnerships with stakeholders and families.
Linking with services including financial assistance, legal aid, housing, medical treatment and other resources.
Supporting clients to engage with services.
Goal setting and problem solving, helping clients to achieve outcomes and support them to successfully transition through life stages.
Case management.

ADVOCACY

Ensuring rights and needs are met.
Support to develop plans to improve client’s well-being.

CRISIS INTERVENTION

Providing support and intervention during crises or emergencies.

CAPACITY BUILDING

Empowering people and building confidence in themselves to develop skills, advocate and manage their own supports.
Support individuals in accessing their community and building friendship and social networks.
Supporting individuals in accessing employment and education.
Coaching and education for families, caregivers and care teams.
Our Social Workers work collaboratively to draw upon experiences and knowledge to develop the solutions together. Our team provide support to comprehensively identify abilities, goals and needs.

EMOTIONAL SUPPORT

Providing emotional support and counselling to clients, their families and care teams is a key aspect of our social work services. This is fundamental to helping individuals resolve, live with and move through social and personal problems.
This support includes therapy for grief and loss, trauma, emotional regulation and anger management. All supports provided are evidence based and tailored to the individual’s needs.

ORGANISATIONAL TRAINING

We offer consultation to organisations and tailor-made staff training including:

supporting emotional regulation and capacity building for clients
Understanding the NDIS
The importance of reporting
Education on data and the need for quality data and data analysis
Navigating systems including Government and Non-Government organisations
